S + V to have (have, has) + been + V1 ing

He has been writing a letter to his friend for 3 hours. 
Passive Voice
S + V to have (have, has)+ been + being + V3

A letter has been being written (by him).

Present Perfect Continuous Negative Form
Active & Passive Voice 
Active Voice 
S + V to have (have, has) + not + been + V1 ing

He hasn't been writing a letter to his friend for 3 hours. 
Passive Voice
S + V to have (have, has) + not + been + being + V3

A letter hasn't been being written (by him).
